This is the core of the build. It’s a silent close-quarter weapon, and the Milano SMG is a close-quarter weapon as well.
The knife will allow you to save ammo and rack up silent kills. Moreover, some unique and Legendary quiet knives, such as the Phlebotomist, are incredibly powerful. A single shot can throw down an enemy.
The Stim is a safe choice for almost any build. You would know it as a steam-pack on any other game. It can regenerate some of your health on use. It’s necessary if you plan to fight in close quarters often, as you will take a lot of damage.
Perk 1: EOD
For perks, we’re starting with EOD. This is a choice for survival, as it will reduce the damage of explosives unless these explosives are part of a kill streak. Consider that most players will fight close-quarter enemies with grenades.
Perk 2: Overkill
Overkill lets you pick two primary weapons on your loadout. So, for example, you’d be able to take the Milano, an assault rifle, a long-range gun, or a shotgun – we recommend an assault rifle, though.
Perk 3: Amped

Laser Optic: Tiger Team Spotlight or Microflex Led
Some people do wonders with the Milano’s standard iron sights, but if you want to take another one, pick the Tiger Team Spotlight.
This attachment will increase the range when enemies become hostile. In other words, it increases the reveal distance. In zombies mode, it will increase the Salvage drop chance.
Another good pick is the Microflex LED. This is a Red Dot/Reflex optic with a low magnification (1.25x). It’s the lowest magnification of any optic class, so it doesn’t really deliver a significant advantage. However, it makes the weapon easy to use, and most players are used to the red dot sights.

We start by choosing the Semtex as your lethal extra equipment. This is a deadly sticky grenade. You can attach it to any surface, even enemies, and it will explode after a short time.
Tactical: Heartbeat Sensor
The next item is a Heartbeat Sensor. As you’re playing in close quarters, this device will help you check enemy positions with a tablet. So, before you rush to a kill, you can study their movement and position.
Perk 1: Double Time or Cold Blooded
The main perk for this build is Double Time. It’s a straightforward choice that lets you sprint twice as fast for a short time. It also increases your crouch movement speed. It will make it easier to catch your enemies, sneak, and run away.
Another choice that matches this build is Cold Blooded. This perk will make you harder to detect. Also, most AI targeting systems won’t find you. All of these will allow you to chain sneak take-downs.
Perk 2: Overkill
Our next pick is Overkill, which we already explained. It will allow you to pick two primary weapons on your loadout.
Perk 3: Combat Scout
The last per is Combat Scout. This ability highlights enemies you hit in orange. Your entire squad can see the mark, even through obstructions and walls.

However, it will negatively affect your walking aim speed. Still, we’re working towards a long-range solution, so this should not be an issue.
Alternatively, the Field Agent Grip will improve horizontal and vertical recoil, but it lowers movement speed as you shoot. It would be best to take cover and shoot from a distance if you pick it.
Laser Optic: Axial Arms 3x
This Optic attachment became available with the Black Ops Cold War to merge. It will give a mil-dot sight and a 3x magnification level. It’s great for medium and long-range combat.
Stock: Commando Foregrip or Raider Stock
The Commando Foregrip improves recoil stabilization and aiming stability at the cost of movement speed. That makes it a good choice for long-range assaults.

This drum offers a more prominent magazine and a bigger ammunition capacity. You will lose ADS time and reload speed, so you’ll need to be extra accurate with your shots.

The Best Companion Weapon for the Milano SMG
The best weapon to take alongside the Milano is the Bruen MK9. This is in case you have the Overkill perk, as this MK9 is also a primary weapon.
The Bren shines because of its long range and accuracy. It’s an assault rifle, which belongs to the most favorite Warzone weapon category.
Some nerfs downgraded the weapon’s power, but it’s still as reliable as it is successful.

The Best Alternative Companion Weapon for the Milano SMG
If you’re taking the alternative setup for the Milano, then it would be best to take a long-range rifle. We recommend the ZRG 20mm, which is hard to use but is deadly.
This sniper rifle offers a better range than the Pearlington 704 or the Kar98k. It comes with three bullets per magazine, and each shot can take down an enemy (if you hit on the torso or the head).

How to Unlock the Milano 821 SMG?
The Milano 821 SMG is easy to unlock. You don’t need to complete any specific challenge. You can open it by playing the game until you reach level 13.
Part of the fame of this weapon is how easy it is to find and unlock. You can get it quickly in a few games. After you reach level 13, you will see it on the inventory, available to pick before you enter the maps.
As it is, though, without any attachments, the weapon is pretty rough. It hits moderately hard but has significant recoil, a low range, moderate bullet speed, and a noisy barrel.
If you must take it to the battlefield without any attachments, ensure to pack a Stim and your shields at all times. You’d also better sneak around and try to take out your enemies by shooting them in the face. Without attachments, it’s still a good choice for maps and game modes with fewer people.
